I understand that I am applying for in-network insurance through Blue Cross Blue Shield of Utah, and that if approved, I or my designee will need to be physically present at Matia Mobility’s Salt Lake City office to inspect and sign for my device. I also understand that if I want to ship my device once I or my designee has signed for it, that Matia Mobility may bill me to crate and arrange shipping on my behalf.
